Senior/Disabled Half-fare Taxi coupons – Fremont Township has contracted with American Taxi to provide ½ fare taxi coupons for Seniors and Disabled Residents. The goal of this program is to provide transportation to areas that our Dial-A Ride buses do not service. Coupon books are purchased at the Township Office for $15.00 and are worth $30.00. American Cancer Society Road to Recovery Transportation Referral – Volunteer drivers donate their time and the use of their cars so that patients can receive the life-saving treatments they need. If you or your loved one needs a ride to treatment, call the American Cancer Society at 1-800-227-2345 to be matched with a volunteer, or search online for programs in your area. Catholic Charities Lake County Senior Services Transportation Grant provides rides to Lake County seniors (60+) to attend medical, dental, vision and hearing appointments and to also pick up prescription medication from the pharmacy. They ask that the senior contact them 5-7 business days prior to their appointment to request services. Transportation is provided Monday through Friday, no weekends, no holidays. The service hours begin at 8:00am (at home pick up) and the senior must be dropped off at their home by 5:00pm. The transportation vendors utilize wheelchair accessible vans and other vehicles depending on the participant’s needs. The vendors are Licensed, Insured, and Bonded. There is a $6.00 donation asked for each round trip provided. Donations received are put back into the program to help provide transportation to other seniors in the Lake County Area. Seniors will not be denied services if they are unable to donate. They are able to provide limited (1 ride per senior, per quarter) transportation to Milwaukee, McHenry County and the City of Chicago. Though they are able to provide temporary assistance for seniors to and from dialysis and long term physical therapy appointments, they cannot provide long term transportation assistance for these appointments.
